By spying on Cicero, going against the Night Mother's wishes, and betraying the Dark Brotherhood, Astrid violates all five of these rules. It's Astrid's megalomania that ultimately causes the overall downfall of the Brotherhood.

Don't you dare kill Cicero!!! Really it's just a matter of whether you like him or not. I love him - he's my favorite follower, probably my favorite character in the whole game. Since you can pickpocket him and receive higher grade gear at the Dawnstar Sanctuary, it is really not lucrative to kill him.

If you have completed the quest line for the Dark Brotherhood, and Cicerio is still alive at that point, he will indeed become a Follower.

If you let Cicero live, he will join you once again after the Dark Brotherhood main quest is completed. You can use him as a follower, and he will be available in the new Dark Brotherhood sanctuary located at Dawnstar. To be fair, he is a really strong follower, so it is wise to let him live.
